Self Referral
Welcome to our self referral tool, from here you can gain access to Herefordshire and Worcestershire Perinatal Pelvic Health Service.
This referral form is to be completed only if you are within the perinatal period (i.e: currently pregnant or within one year postnatal). It is for those with symptoms of pelvic girdle pain (pain in or around the pelvic area), rectus abdominis (abdominal separation) or pelvic floor dysfunction such as
- Accidental loss of urine
- Being unable to make it to the toilet on time
- Feelings of increased pelvic pressure or sensation of your pelvic organs slipping down or falling out
- Pain with sexual intercourse
- Accidental loss of wind or poo
If you do not meet these criterial you will need to be referred via your GP or other health care professional to the appropriate service.
